What data we use and why
The form collects your name, telephone number, reason for your enquiry and optional message. We use this data to receive, manage and respond to your request by telephone or using the contact details you provide. Your enquiry is sent directly to ypd@estomatologo.es without opening your email application.
Providing your data is voluntary, but we need your name and telephone number to handle a request through this form. Do not include diagnoses, reports, clinical photographs or other health data. This form is not intended to collect medical information or handle emergencies.
Legal basis
We process enquiry data on the basis of your consent, which you give by ticking the checkbox and submitting the form (Article 6(1)(a) of the GDPR). You can withdraw it by writing to the email address above, without affecting processing carried out before withdrawal. We do not use this consent to send advertising.
The technical processing necessary to protect the website and prevent abusive submissions is based on the legitimate interest in maintaining its security (Article 6(1)(f) of the GDPR).
Recipients and retention
Enquiries are accessed by people authorised by the controller to manage them. Web hosting and email providers supporting the service, hosted on webd.pl, may process data to provide those services. We do not publish your data. If your enquiry needs to be forwarded to an independent professional or clinic, you will be informed of their identity and asked for your authorisation beforehand.
We retain your enquiry while it is being handled and during the follow-up necessary to address it. It is then deleted, unless limited information needs to be retained to comply with a legal obligation or address potential claims during the applicable legal period. You can request its deletion where appropriate.
Your rights
You can request access, rectification, erasure, restriction, objection and, where applicable, portability, and withdraw your consent by writing to ypd@estomatologo.es or the controller’s postal address. State which right you wish to exercise; proportionate information may be requested where necessary to verify your identity.
You can lodge a complaint with the Spanish Data Protection Agency or the supervisory authority in the place where you live, work or where the alleged infringement occurred. The Polish authority is Urząd Ochrony Danych Osobowych (UODO).
Technical operation and external services
The server processes IP addresses and technical data to serve the page, maintain security and limit repeated submissions. The form does not make automated decisions or profile individuals.
